Boarding Forms – The following forms are required for boarding
- Shot Records
- Boarding Contract (Filled out during check-in)
Daycare Forms – Please click on the form, print and return the completed form to Dog Days Inn along with the shot records.

- Release Agreement (in the application)
Holiday Boarding Form – This form is required for boarding during the upcoming holiday periods, including New Year’s, Easter, Memorial Day, the 4th of July, Labor Day, Thanksgiving, and Christmas.
We require the following shots for the pets:
Dogs
- Bordetella (Every 6 months)
- Distemper
- Rabies
- Canine Influenza
Cats
- Feline Leukemia
- Rabies
